Vista 64 has very few problems with B&W1.
Just do the normal things.
Update Vista to SP1 or above then the same install as for XP or W2k.
As an Administrator........
Install the game... full install.
Patch the game using the v1.2 only.
Apply the Myth v1.2 NoCD patch
Copy the Audio folder from the CD to your main game folder.
In the main game folder find and run Setup(.exe) set the screen Resolution to 800x600 and minimum everything that can't be turned off.
Set the desktop resolution to 800x600.
Shut down...Disconnect from the internet.....Reboot.
Run the game as Administrator.

If the game runs OK now..... You can start adding things back to the game you should be able to run every thing maxed out even in Wide Screen with a little tweak or 2.
If the game is not OK then you have a good place to start trouble shooting.

Just a final word it seems that just a few laptop chipset's will not work with B&W ATM and nothing can be done .
Some manufacturers think the game is not worth supporting any more.
